A 12 yr old girl presented with maculopapular rash with a cephalocaudal distribution; posterior auricular and suboccipital LAP and arthralgia
Dx?
Rubella

Sensory neural hearing loss; cataracts; cardiac defects like PDA
The most common cause of sepsis in pts with sickle cell disease (asplenia)

A 5 yr old boy has been having persistent sxs of nasal drainage, congestion, cough for more than 2wks and has fever, purulent nasal discharge for the past 3days
Dx, most common predisposing factor
Bacterial sinusitis
Viral upper respiratory infection is the commonest predisposing factor; not allergic rhinitis
Unlike adults, in children with suspected GAS pharyngitis what should b done b4 abx?
Throat culture or rapid strept Ag testing
How do u treat syphilis in pts with penicillin allergy?
- Primary?
- CNS involvement, pregnants, multiple Rx failure?
- Oral doxycycline
2. Desensitization n then penicillin
Features of the d/t congenital infections other than the ones common to all( cmv, toxo, syphilis, )
CMV- periventricular calcification
Toxo- severe chorioretinitis, intracerebral calcification
Syphilis- rhinorrhea(snuffles), abnormal long bones, desquamating bullous rash
Drugs of choice for hep B(2dgs)
Entecavir, tenofovir

Nausea, vomiting, diarrhea after undercooked meat(usu pork) ingestion
Mm stage- myositis, periorbital edema, subungual splinter hemorrhages, eosinophilia (usu>20%)
A woman presenting with sxs of pyelonephritis, hx of recurrent UTI, nephrolithiasis,
U/A shows a PH of 8.5
The most likely causative agent?
Proteus mirabilis- a urease producing bacteria—> generates ammonium—> urine alkalinization—> decreases phosphate solubility—> struvite stones
