Indications for LTOT: (2)
1- arterial PaO2<55 mmHg or an arterial oxygen saturation SaO2<88%
2- arterial PaO2<60 mmHg or an arterial oxygen saturation SaO2<90% AND either cor pulmonale, RHF, or erythrocytosis (Hct>55%)
Indications for prescribing oxygen during sleep: (3)
1-PaO2<55mmHg
2-Pulse Ox (SaO2)<88%
3-PaO2 falls 10 mmHg or more during sleep AND sxs of nocturnal hypoxemia (insomnia, restlessness, or impaired cognitive process)
Clinician Responsibilities in prescribing LTOT: (8)
1– Proper determination of need
2– Determining the oxygen flow rate for each condition (ie, the required flow rate may be different during sleep, exercise and sedentary
wakefulness)
3– Assuring the selection of a qualified durable medical equipment supplier
4– Completion of the Certification of Medical Necessity (CMN)
5– Providing a prescription for ambulatory oxygen if required
6– Testing patients using the portable oxygen device provided during
typical exercise to assure adequate oxygenation
7– Reassessment for prescription renewal and/or modification
8– Patient education and training
Recommended FIO2 and nasal cannula oxygen settings to achieve ambient air PaO2>60mmHg with PaO2 levels of: 50mmHg, 45mmHg, 40mmHg, and 35mmHg:
PaO2 50mmHg: 24 FiO2: 1 L/min
PaO2 45mmHg: 28 FiO2: 2 L/min
PaO2 40mmHg: 32 FiO2: 3 L/min
PaO2 35mmHg: 36 FiO2: 4 L/min
