Diseases of the larynx
- Inflammatory conditions
a. Laryngitis
b. Laryngoepiglottitis - Reactive nodules

- Spindle cell carcinoma
- Verrucous carcinoma
- Mucoepidermoid carcinoma
- Adenoid cystic carcinoma
Causes of laryngitis
- Viral
- Bacterial (Corynebacterium diphtheriae which produces
membranous inflammation) - Tuberculosis
Causes of laryngoepiglottitis
- Haemophilus influenzae
- Respiratory syncytial virus
- Beta-hemolytic streptococci
Complications of laryngoepiglottitis
Sudden swelling of epiglottis & vocal cords obstructing airway (mainly in young children)
Singer’s nodules
- Occurs in individuals who use their voices extensively
- A polypoidal lesion on true vocal cords
- Characteristically changes character of the voice & often
cause progressive hoarseness - Non-neoplastic (do not give rise to cancers)
Squamous cell papilloma
- Benign tumour, may be single or multiple
- Caused by human papillomavirus (HPV) types 6 & 11
(especially recurrent multiple papillomas) - Complications: obstruction, progression to squamous cell carcinoma (rare)
Epidemiology of Squamous cell carcinoma
- Common over the age of 40

Pathogenesis of Squamous cell carcinoma
Step-wise progression of tumour: epithelial dysplasia of varying degree → carcinoma-in-situ → invasive carcinoma
Classification of Squamous cell carcinoma based on location
- Glottic: confined to one or both cords (slow to metastasize, good prognosis)
- Supraglottic: involves the epiglottis, false but not
true cords (metastasis more common than glottic) - Subglottic: located below the cords (late presentation, poor prognosis, extensive growth leading to stridor & voice loss)
Growth patterns of Squamous cell carcinoma
Polypoidal &/or ulcerative growth patterns, commonly demonstrating local invasion
Spead of Squamous cell carcinoma
Spread to regional lymph nodes & distant metastases can occur
Other malignancies of the larynx
- Spindle cell carcinoma (resembles a sarcoma)
- Verrucous carcinoma
- Variant of squamous cell carcinoma
- Locally destructive, may cause obstruction - Mucoepidermoid carcinoma
- Adenoid cystic carcinoma
