16
Q

Effects of NSAID (ibuprofen) on kidney?

A

raised urea, creatinine and potassium!!!!!!

Ibuprofen decreases renal blood fow by inhibiting prostaglandins that normally dilate blood vessels flowing into the kidney, especially if the patient has renal artery stenosis. Reduced blood flow to the kidney mimics hypotension and prerenal failure, which if prolonged causes intrarenal failure

17
Q

Effect of Erythromycin on INR?

A

erythromycin is an enzyme inhibitor, i.e. reduces
warfarin breakdown and thereby increases its plasma level, INCREASING INR

18
Q

What is neutropenic sepsis?

A

Any source of sepsis (in this case pneumonia) with neutrophils <1

19
Q

ABx for neutropenic sepsis?

A

START PIPERACILLIN WITH TAZOBACTAM IV, GENTAMICIN IV AND PARACETAMOL

20
Q

how does carbamazepine effect sodium?

A

carbamazepine can also cause
hyponatraemia (through SIADH)

21
Q

What condition to with hold digoxin?

A

Slow AF

22
Q

Circumstances to increase steroids?

A

When Addisonian patients become sick they should increase their intake of steroids (so-called ‘sick day rules’)
to provide adequate cortisol for the stress response

23
Q

Drugs that can trigger an asthma attack?

A

Ibuprofen is an NSAID which can trigger an asthma attack
Bisoprolol is a beta-blocker. Its side effects are bronchospasm (which would cause wheeze)

24
Q

What circumstance would you withhold salbutamol inhaler?

A

When also using nebulized salbutamol

25
Q

Treatment of acute heart failure?

A

40mg IV Furosemide

26
Q

What is fast AF?

A

AF with HR >100bpm

27
Q

first line Abx for gut infections?

A

Metronidazole is typically used for gastrointestinal infections because of its good effects on anaerobes which
colonize the gut

28
Q

Skin infection first line Abx?

A

FLUCLOXACILLIN 500MG, 6-HOURLY FOR 7DAYS

29
Q

Abx for severe celluitis?

A

FLUCLOXACILLIN 1G, IV STAT

30
Q

Abx for bone infections?

A

E CLINDAMYCIN 500MG, 6-HOURLY FOR 7DAYS

31
Q

Two drugs that can cause antimuscarinic side effects?

A

Cyclizine and amytriptiline

32
Q

Side effect of carbimazole?

A

carbamazepine (Gabapentin alternative)
Carbimazole

33
Q

Hydroxocobalamin

A

vitamin b12

34
Q

drugs that can precipitate parkinsonism?

A

Both metoclopramide and haloperidol are dopamine antagonists and can precipitate parkinsonian symptoms, even in patients who do not have
Parkinson’s disease.

35
Q

Antiemetic that can be used in Parkinsonism?

A

It is useful to know that domperidone, another dopamine antagonist licensed for the treatment of nausea, is safe to use for patients with Parkinson’s disease as it does not cross the blood-brain barrier
