2. Atypical (second generation): Risperidone, Olanzapine, Quetiapine, Clozapine, Aripipazole
3
Q
MOA of typical antipsychotic drugs (Haloperidol)
A
Dopamine-specific antagonism (D2 receptor)
4
Q
Adverse effects of typical antipsychotic drugs (Haloperidol)
A
- Extrapyramidal reactions (blocks dopamine)
- Sedation
- Anticholinergic (blocks acetylcholine)
- Rare cases of neuroleptic malignant syndrome
5
Q
MOA of atypical antipsychotic drugs
A
- D2 receptor antagonism (less pronounced than that of FGAs)
- 5-HT2A receptor antagonism
- Interaction with several other receptors (i.e., D3, D4, α-adrenergic, and H1 receptors)
- Aripiprazole is a partial D2 agonist
6
Q
Adverse effects of atypical antipsychotic drugs
A
- Clozapine- agranulocytosis in 2% of patients, weight gain, sedation
- Olanzapine- weight gain and sedation
- Risperidone - EPS (dose dependent), weight gain
- Quetiapine- weight gain
