  3. Submit original copy of the Evidence Collection tracking form to the DOHMH evidence intake area officer
  4. Prepare the property clerks invoice worksheet maintained at DOHMH evidence intake area. List the packaged sealed samples of the unknowned substances and the related evidence items that are being submitted to the DOHMH and catagorize the property as investgatory.
