2.10 Photo, photo, photo, photo Flashcards
Constant throughout the day/year
Long waves penetrate deeply - dermis, subcutaneous fat
Chronic exposure = connective tissue degeneration
What UV is this?
UVA
Greatest during the summer - most harmful of waves
High amt of exposure to corneum/superficial layers
Sunburn/tan and inflammation
What UV is this?
UVB
Absorbed by ozone layer
Transmitted only by artificial sources – germicidal lamps
What UV is this?
UVC
Very sensitive
Always burn easily; never tans
Type I
Very Sensitive
Always burns easily; tans minimally
Type II
Sensitive
Burns moderately; tans gradually and uniformly (light brown)
Type III
Moderately sensitive
Burns minimally; always tans well (moderate brown)
Type IV
Minimally sensitive
Rarely burns; tans profusely (dark brown)
Type V
Insensitive
Never burns; deeply pigmented (black)
Type VI
Definition: Skin changes superimposed on intrinsic aging by chronic sun exposure
Photoaging
Sun damaged skin is characterized by?
Elastosis (a coarseness and yellow discoloration of the skin)
Irregular pigmentation
Roughness or dryness
Telangiectasias
Atrophy, deep wrinkling, follicular plugging, and a variety of benign and malignant neoplasms
What is Thickened, wrinkled, yellowish skin – forehead/back of neck?
Solar Elastosis
What is Thinned skin, prominent blood vessels, easy bruising, etc?
Atrophy
What is Deep, do not disappear by stretching the skin.
Wrinkles
What is sun-induced wrinkling on back of neck that forms a rhomboidal pattern?
Cutis Rhomboidalis Nuchae-
What are the Photoaging Vascular Changes?
1) Diffuse Erythema - Fair-skinned individuals
2) Ecchymoses and Stellate pseudoscars
§Bleeding into skin following minor trauma
§Exposed surfaces of back of hands and arms
§Atrophy, ease of skin tearing
3) Telangiectasias - Cheeks, nose, ears
4) Venous Lake - Round, purple ectatic vessels, lower lips and ears
What is Red-brown reticulated pigmentation with telangiectasias, atrophy, prominent hair follicles. Chest and neck?
Poikiloderma of Civatte
What are the Photoaging Papular Changes?
- *Nevi** - more numerous on sun-exposed surfaces
- *Seborrheic Keratosis** - more numerous in sun-exposed areas
- *Yellow Papules (solar elastosis)** - dull-to-bright yellow papules that may coalesce to form plaques
- *Favre Racouchot** - Comedones and cysts around the eyes
Photoaging: Tx
Topicals: Retinoids
Slow to produce change (3-6 months)
§Increased photosensitivity during tx = need daily sunscreen use
Treats fine wrinkles, pigmentation changes, tactile roughness (not effective for course wrinkling or telangiectasias – vascular changes)
Resurfacing: Chemical peels, Dermabrasion, lasers
Faster results.Mild/Significant perioperative morbidity
Complications: Scarring/Hypopigmentation
