LVADs wanted flow arrythmias atrial ventricular arrest
3-5 L/min
common - especiall in first 30 days - LVAD keeps flow despite arrythmia.
atrial - beta blockers rate control best
ventricular - shock - pads away from LVAD
CPR can be appropriate- may dislodge LVAD - but if unconscious and no MAP or whirl - then do it
AvR elevation
only a worry if diffuse ST depression elsewhere
global disease or prox LAD.
or global strain - dissection, PE, anemia, adrenalin
calcium dosing in ACLS in NZ
10ml of 10% for both CaCl and Cagluconate
Mag dosing conversion
1g is 4mmol
ketamine drip post sedation
20mg per hour
potassium replacement in NZ
chlorvescent - 14mmol - in water
Kspan - mod release - 2 tablets is 16mmol
IV - 20mmol in IVF over 2 hours peripherally
bleeding trach
take any bleed seriously - no DC - CTA?
first step is inflate cuff with 50cc of air (if pilot balloon)
how old is trach?
last ditch effort is to remove and place pressure with finger
flipped T wave in AvL
in active chest pain can be first sign of inferior wall ischemia
