(b) You are an OCE (open cut examiner) on day shift and receive a 2-way call notifying you that a maintenance fitter has completely severed his index finger while replacing a bucket tooth on an excavator in the pit. Summarise the actions you will take as an OCE to manage this incident and fulfil your legal requirements. (14 marks)

– Serious injury/illness (amputation)
+ make sure the fitter receives the appropriate first aid from site emergency response personnel.
+ make sure that professional medical advice is both sought and followed and that external emergency services are contacted immediately.
+ Notify MEM and the fitter’s next of kin of the incident.
+ As its a notifiable incident, the resources regulator and ISHR will need to be notified immediately after becoming aware the incident has occurred by the fastest possible means (phone) and in writing within 48 hours.
+ Usually the MEM would make the notifications but if they cannot be contacted I as the OCE may be required to make the notification.
+ The scene, including the excavator and any equipment in the immediate area, will need to be isolated and preserved so far as is reasonable practicable until an inspector arrives or a government official releases the scene at an earlier time.
+ begin preliminary investigations (statements, photos, D&A testing etc).
