2018 Start Flashcards
Used in times of starvation or stress
Gluconeogenesis(mechanism by which lactic acid and amino acids converted to glucose)
Catalyze the formation of thrombin
Prothrombin complex
Vitamin K dependent factors
II, VII, IX, and X, protein C and S (2, 7, 9, and 10)
Long bleeding time (ristocetin test)
von Willebrand’s disease
von Willebrand’s disease treatment
I - reduced quantity most common; DDAVP II - defect in vWF; Cryoprecipitate, DDAVP III - complete vWF deficiency; Cryoprecipitate
Hemophilia A
Sex-linked recessive, most common symptom hemarthrosis, prolonged PTT, factor VIII crosses placenta so newborns might not bleed at circumcision;
Cryoprecipitate
Treatment for bleeding in uremic patients
1st - Hemodialysis
2nd - DDAVP (acute reversal)
3rd - Cryoprecipitate
Heparin induced thrombocytopenia (HITT)
Secondary to anti-heparin antibodies treat with stopping heparin and starting argatrogan (direct thrombin inhibitor)
Stop ASA ___ days preop
7 days
Stop plavix ___ days preop
7 days
If needed, bridge with Integrilin (eptifibatide)
Stop Coumadin ___ days preop
7 days
Consider heparin bridge; tx bleeding with PCC, Vit K and FFP
Goal platelet count
> 50K preop and >20K postop
Aminocaproic acid
Bleeding after circumcision
Hemophilia (?)
99% of bleeding disorders identified this way:
Abnormal bleeding with tooth extraction or tonsillectomy
IVC Filter indications
Contra indications to anticoagulation PE while on anticoagulation Free-floating IVC, iliofemoral, or deep femoral DVT Recent PE Pl. IV see below the renal veins
Best diagnostic study for PE
CTPA (?)
Used in DIC, persistent bleeding following Cardio pulmonary bypass, or thrombolytic overdoses
Aminocaproic acid (Amicar) - Inhibits fibrinolysis by inhibiting plasmin
MOA for dabigatran, apixiban, argatroban, and rivaroxaban
DOACs - direct thrombin inhibitors
Use argatroban with HITT
LMWH (Lovenox, enoxaparin)
Vines and activates anti-thrombin III but increases neutralization of just factor XA not reversible with protamine
Treatment for thrombolytic overdose
Aminocaproic acid
Back pain, chills, tachycardia, fever during transfusion
Acute hemolytic reaction
Tx: Stop transfusion, fluids, diuretics, bicarb, pressors
Best way to avoid blood transfusion reactions
Type and cross
Electrolyte abnormality with massive transfusion
Hypocalcemia
Effector cells in cell mediated immunity
Macrophages, cytotoxic T cells, natural killer cells
Primary effector cells in cell-mediated immunity
Cytotoxic T cells (?)
What medicines cause degranulation of mast cells? Which do not?
IgE complexes, complement components, mechanical trauma, endotoxins, and free radicals.
Not degranulated by corticosteroids (?)
Most common anaerobe in the colon?
Bacteroides fragilis
Most common aerobic bacteria in the colon
E coli
Most common bacteria overall in the colon?
Bacteroides fragilis (?)
C Diff treatment
PO Vanc or flagyl; IV flagyl
___% of abscess have anaerobes
___% of abscess have anaerobes and aerobes
90% of abscess have anaerobes
80% of abscess have anaerobes and aerobes
