Pharmaceutical Care

Pharmaceutical Care
A review of the medical conditions and symptoms to determine the need for drug therapy.
Assessment
A decision of an appropriate drug therapy based on the assessment of the patient.
Plan
A review of the outcomes of drug therapy to determine if the patient is obtaining the desired outcomes.
Monitoring
A distinct service or group of services that optimize therapeutic outcomes. Patient centered assessment and evaluation of the patient’s full medication.

Medication Therapy Review
Purposes of Medication Therapy Review (MTR)
To improve patients’ knowledge of their medications; Address problems or concerns that patients may have; and Empower patients to self-manage their medications and their health condition(s).
A comprehensive record of the patient’s medications (prescription and non-prescription medications, products, herbal and other dietary supplements).
Personal Medication Record (PMR)
Purposes of Patient Medication Record (PMR)
To communicate and collaborate with physicians and other healthcare professionals; Widespread use of the PMR will support uniformity of information provided to all healthcare professionals; and Enhance the continuity of care provided to patients.
A patient-centric document containing a list of actions for the patient to use in tracking progress for self-management.
