Opioid misuse
Acute OD - IV/IM naloxone
Detox - methadone/buprenorphine - 4 weeks inpatient or 12 weeks outpatient
Causes of lung fibrosis
Amiodarone, methotrexate, nitrofurantoin
Contraindications of sildenafil (viagra)
Nitrates, nicorandil, hypotension, recent stroke/MI
Drugs with zero order kinetics
Phenytoin, salicyclates, heparin, ethanol
Drugs affected by acetylator status (being dificient in hepatic N-acetyltransferase)
Isoniazid, procainamide, hydralazine, dapsone, sulphasalazine
CYP450 inducers
Phenytoin, carbamazepine, phenobarbtone, rifampicin, St John’s Wort, chronic alcohol intake, griseofulvin, smoking
CYP450 inhibitors
Ciprofloxacin, erythromycin, isoniazid, cimetidine, omeprazole, amiodarone, allopurinol, ketoconazole, fluconazole, fluoxetine, sertraline, ritonavir, sodium valproate, acute alcohol intake, quinupristin
Clenched jaw and eyes are deviated upwards caused by a medicaiton
Oculogyric crisis
Due to phenothiazines, haloperidol, metoclopramide
Mx - IV antimuscarinic (benztropine/procyclidine)
Neuropathic pain
Amitriptyline/pregabalin –> amitriptyline + pregabalin –> tramadol + refer to pain specialist
Diabetic neuropathic pain
Duloxetine
Heparin
IV, short duration of action, monitored with APTT, activates antithrombin III, can cause hyperkalaemia
LMWH
IM, long duration of action, monitored with anti-factor Xa, activates antithrombin III, can cause hyperkalaemia
Heparin OD
Protamine sulphate
Lithium toxicity can be precipitated by
Dehydration, renal failure, diuretics (bendroflumethiazide), ACE inhibitors, NSAIDs and metronidazole
