2/3/20 - Antiarrhythmic Drugs Flashcards
What is the name of the classification of AAR drugs?
Vaughan Williams Classification
MOA of classes?
Class 1 - Na blockade
Class 2 - Beta Blockers
Class 3 - K blockade
Class 4 - Ca2+ blockade
Give 3 examples of class 1a drugs and what is their potency?
Disopyridamine
Quinidine
Procainamide
Medium strength
Indication and SE of procainamide?
Wolff-Parkinson-White syndrome (accessory bundle of Kent and delta wave - slurred upstroke of Q wave - on ECG)
Drug induced lupus erythamotousis syndrome (along with HIPP drugs - hydralazinee, Izoniaszid, Procainamide, Phenytoin)
What blood test could be done to investigate for a SE of procainamide?
Anti histone antibodies (or anti nuclear antibodies) to investigate for Drug indiced Lupus (reversible)
SE of quinidine
Cinchonism (tinnitus, blurry vision and headache) - word derived from cinchona bark
Disopyridamine SE and contraindication
Anticholinergic SE (dry mouth, blurry vision, urinary retention, hypotension)
Contraindicated in heart failure as negative inonotropic effect
Class 1a AAR SE
QT prolongation and torsades de point
Two example drugs from 1b and their potentcy
weakest of class 1 drugs
lidocaine and mexilitine
Indication and SE of lidocain
Digoxin toxicity, SE - CNS toxcity
SE of Mexilitine
GI disturbance
2 examples of Class 1c and its strength
Strongest of class 1
Flecanide
Propafenone
How long after and MI can you not drive for?
4 weeks
What drugs have been proven to improve long term mortality in heart failure and for each name a drug trial
ACE inhibitors - SAVE, CONSENSUS B Blockers - CIBIS Spironolactone (Aldosterone blockers) - RALES Angiotensin Receptor Blockers Hydralazine with nitrates - VHEFT-1
What is a normal corrected QT interval in males and females
430ms in males and 450 in females (each small sq = 40ms therefore over 11 small sq is concerning)
