STOPP: Screening tool of older people’s prescriptions
START: screening tool to alert to right treatment

No because they only have access to a summary care record which does not include fraility score and no access to system one

For each anticholinergic a person is taking increases the risk of cognitive impairment by 46% in 6 years

What are the seven steps to medication review?

- Assess patient
- Define patient goals
- Identify inappropriate drugs using NO TEARS
- Assess each drug for risks vs benefits
- Continue, Stop or Reduce dose
- Communicate with other healthcare practitioners
- Monitor and review

What are some reasons we need to review elderly people’s medications more often?
- Declining renal function
- Reduction in receptor sites as you get older
- Adverse drug reactions more common
How can you work out a patient’s frailty score initially?
- Cut off score of >3 needs further investigation for frailty
- Look at Rockwood Frailty Score
