What is nerve one
A
- Ofactory,
sense of smell.
Tested by having patient close eyes and presenting them with smells.

What is nerve two
A
- Optic nerve, vision. Tested using melon chart or confrontation test; crossarm across body cover same eyes patient move hand into field of vision.

What is nerve 3
A
- Oculomotor nerve, eye movement up and pupil movement. PERRLA- pupils, round, reactive to light, look at size, accommodation for things approaching. EOM: extra ocular movement. Have patient follow finger in all directions six fields, touch face.

What is nerve number 4
A
- Trochlear nerve, medial and downward eye movement. Same test used in third nerve.

What is nerve number five
A
- Trigeminal nerve, jaw movement, sinus, face, teeth feeling. Tested by having patient clench jaw and feel muscles on the side tighten up.

What is nerve number 6
A
- Abducens nerve, eye movement lateral. Tested in the same way as nerve three.

What is nerve number 7
A
- Facial nerve, muscles for facial expressions. Tested by asking patient to smile frown lift eyebrows close eyes and resist from having them opened, puff cheeks and resist trying to have them pushed out.

A
- Veatibulocochlear nerve, hearing and balance. Whisper test.

What is nerve number nine
A
- Glossopharyngeal nerve, anterior 1/3 of taste bud. Uvula

What is nerve number 10
A
- Vagus nerve, throat muscles, swallowing and abdominal nerves.

A
- Accessory nerve, motor control to trapezius muscle, neck movement

What is nerve number 12
A
- Hypoglassel nerve, tongue movement. Tested by having patients stick tongue out and then have them say light tight dynamite.
