Screening
- Is the client appropriate and eligible for admission into program
Appropriateness i.e. inpatient, outpatient, residential, detox, etc.
Eligibility I.e. age, gender, place of residence, legal status, veteran status, income level, referral source etc.
- Identify coexisting conditions that may require additional services
- Adhere to state laws and agency policies i.e. Hippa Laws
- if pt is found ineligible the counselor should suggest alternatives i.e. referrals, minimum of 3
Intake
- Administrative and initial assessment procedures for admission into program
- Complete required documents for admission
- Complete documents for eligibility and appropriateness ( financial data and level of care)
- Obtain signed consent forms to include consent for tx
This process is an extension of the screening DECISION TO ADMIT IS FORMALLY MADE.
Orientation
-Describing to the client:
Nature and goals of the program
Client Obligations
Rules about conduct and infractions that can lead to disciplinary actions or discharge
(Non-residential) hours of service availability
Tx clost
Client rights
May include other professionals such as those explaining medications
- This process can occur before, during, ot aftrr client screening and intake in an individual, group, or family context.
Assessment
- Counselor identifies clients strengths, weaknesses, problems, and needs in order to create tx plan.
Gatger historical info:
Demographics
Hx to include SA history
Identify methods for obtaining corrobative info regarding SA hx and other hx i.e. significant others, family, friends, etc
- Identify assessment tools if needed and explain to client the rationale for use
- Diagnostic evaluation based on client needs
Assessment is a continual process and usually the info is gained through interview, assessment tools, or record review. Counselor assesses how significant SA has afffected the clients life. Assessment identifies focus on tx.
Tx Planning
- Counselor and client
identify and rank problems
develop agreed upon immediate and long term goals ( actions client will take)
decide upon a tx methods and resources to utilize. - Explain assessment tool results
Plan is based on assessment and tailored to individual needs, regularly reviewed/modified,
should inlude:
Statement of problem
Objectives to resolve problem
Goal ( expressed in bx terms in order to assess progress or lack of)
The plan/intervention is specific activity that links the problem with the goal i.e. services, frequency, who will perform them, etc.
Counseling
- Individual, group, and significant others
- Skills implemented to assist individuals, families, or groups in achieving objective
- Examination of attitude and feelings
- Decision-making
- Considerations for alternative solutions
- Help client utilize resources to resolve problems
During this process:
- Select counseling theory
- Apply techniques and explore problem and consequences
- Interact with client in a respectful way in which boundaries are maintained
- Elicit solutions BUT ALWAYS PUT THINGS BACK ON THE CLIENT
- Implement tx plan
MUST BE ABLE TO EXPLAIN RATIONALE FOR COUNSELING APPROACH OR WHY THE APPROACH WAS CHANGED
