10 - cestodes Flashcards

1
Q

Q

What is the difference between the scolex on pork tapeworm (T. Solium) and beef tapeworm (T. Saginata)

A

The pork tapeworm has 4 large suckers and a rostellum containing a double row of hooks, whereas the beef tape worm only has 4 suckers

3
Q

Each proglottid is monoecious, what does this mean?

A

The proglottids contain both male and female sex organs (I.e both testes and ovaries)

Q

What happens once a proglottid has been fertilised?

A

It fills with eggs and the gravid proglottid breaks off the chain and passes out in the faeces

Q

What is the difference between the gravid proglottids of pork (T. Solium) and beef (T.saginata) tape worm?

A

T. Saginata has more uterine branches than T. Solium

Q

How do cestodes get nutrients?

A

They absorb nutrients through a specialist transport membrane in the tegument. The tegument has microvillous projections that increase the surface area

Q

How do cestodes excrete waste?

A

Cestodes have protonephridium which consists of flame cells and tubules. Cillia in the flame cells beat, which generates a current and forces water containing nitrogenous waste into the tubules. The waste the, moves out of the tubules to the external environment via excretory pores

Q

How does a cysticercus develop?

A

When oncospheres obtained from the ingestion of tape worm eggs passes to tissues (via the blood) from the duodenum. The oncospheres then encysts within tissues and an embryophore (a thick membrane) surrounds the oncosphere to protect it

Q

How is pork and beef tapeworm transmitted between humans and pigs/cows?

A
  • eggs or gravid proglottids from an infected human are passed out in the faeces
  • pigs/cows then become infected by eating vegetation that is contaminated with the egg or gravid proglottids
  • oncospheres then hatch and migrate to muscle tissue where they develop cysterceri
  • humans are then infected when they eat raw or undercooked meat from infected animals

Q

What is the global distribution of taenia saginata?

A

Worldwide distribution, but more common in Africa, Eastern Europe, the Philippines and Latin America. It is very rare in India as Hindus don’t eat beef.

Q

What is the global distribution of Taenia Solium?

A

Worldwide distribution but it’s most common in regions where humans live in close contact with pigs and eat undercooked pork. It is rare in Muslim countries as they don’t eat pork.

Q

What symptoms do humans have when infected with taeniasis?

A
  • mild symptoms such as abdominal pain, nausea and weight loss
  • psychological distress due to passing motile segments
  • in rare cases, there may be intestinal blockages

16
Q

How is cystercosis diagnosed?

A

By demonstration of cysticeri in tissues

17
Q

How is neurocysticercosis treated?

A
  • symptom management (such as anti-epilepsy drugs and treatment for increased intracranial pressure
  • praziquantel (often co-administered with corticosteroids as praziquantel can provoke an inflammatory response p)
18
Q

What is the structure of an adult dog tapeworm?

A

A 3-6mm worm consisting of a scolex, short neck and 2-3 proglottids

19
Q

What is the global distribution of echinococcus granulosus (dog tape worm)?

A

It is found worldwide (except in North America) but is highly endemic in areas of South America, Northern Africa and areas of Asia and Europe

20
Q

What is the structure of a hydatid cyst?

A

A thin germinal layer surrounded by a thick laminated layer. Within the germinal layer, there are brood capsules which contain protoscoleces which can either stay in the capsules or burst out into the cyst fluid

21
Q

How do hydatid cysts cause pathology?

A

They enlarge and exert pressure on organs such as the lungs, liver and brain

22
Q

How are hydatid cysts diagnosed?

A

Through imaging such as ultrasound, CT or MRI

23
Q

How are hydatid cysts treated?

A
  • PAIR (puncture, aspiration, injection of hypertonic saline and re-aspiration)
  • Surgery
  • Drug treatment (albendazole)
  • watch and wait
