Mid-40s or older

What causes it?
Gradual breakdown of cartilage between joints
Inflammation, joint swelling
‘Wear and tear’
What risk factors are there?
Obesity Trauma Occupation / joint overuse eg labourers Smoking Family history
How does it present?
Joint pain and stiffness Joint swelling / feeling 'knobbly' Crackling sensation Weakness and muscle wasting Limited range of movements Knees and hips primarily affected
What signs may the patient have on examination?
Weakness and muscle wasting
Limited range of movements
Mild swelling and tenderness
Crepitus
What other conditions might present similarly?
RA
Bursitis / tendonitis
Polymyalgia, SLE
Malignancy
How would you investigate?
FBC, RF
X-ray
What treatments are there?
Lifestyle modifications (weight loss)
Pain relief eg NSAIDs, Duloxetine
Surgery if severe
What complications are there?
Chondrolysis
Bone death
Stress fractures
Falls risk
