By submitting a copy of the new CG-4170 forwarded to HRSIC

Annually in the months of October and November.

Upon initial entry, when any info changes in block 6—16, recall to active duty, breaks in service, retirement, and upon reporting to a new PDS.

Active duty, reserve, retired, and cadet mbrs.
How many copies of the CG-4170 are submitted?
Original plus 2 copies.
How should an attached sheet to the CG-4170 be labeled?
Attached sheet for record of emergency date for (mbr, (ssn), dated (date).
Who signs the attached sheet to the CG-4170 if the sheet designates any beneficiaries?
The mbr and a witness.
Who is a casualty reported to by msg?
To the district that the Next of Kin resides in (The district CO must then notify the NOK).
Who is the primary next of kin?
Legal spouse then eldest child over age 21 then father/mother then brother/sister, then next blood relative.
How is the primary next of kin notified?
They will be personally contacted and advised of all rights, benefits, and privileges they may be entitled to.
How long does the CO have to write a letter to the spouse/parents/others (as applicable) in cases of death or serious illness or injury?
With 48 hours of occurrence

When the person has been in loco parentis status for 5 or more continuous years at any period before the mbr turned 21.
Upon a mbrs death how long can his/her depns collect BAH?
Up to 180 dys.
What is a single mbr paying child support in an amt that exceed the difference btwn bah II with and Bah II w/o eligible to receive?
BAH-Diff (equal to the difference btwn w and w/o BAH II and the mbrs grade).
Can a mbr live in non-family type gov’t quarters and still receive BAH-Diff?
Yes.
What is the grandfathering clause for certain mbrs receiving BAH with solely by reason of child support?
Mbrs paying child support equal to the amt of BAQ at the with rate fo the mbrs grade were entiteld to receive BAH with prior to 4Dec91 These mbrs could still live in gov’t quarters and continue to draw the BAH. This exception expires once a mbr becomes entitled to BAH w/ for any other reason
Can a mbr receive BAH-Diff and BAH-Partial at the same time?
No.
When is a mbr never entitled to BAH-Diff regardless of any other situation?
When assigned to family type quarters.
If a mbr takes no cost orders to a new PDS in the same MHA as the old PDR does the mbr receive BAH rate protection?
No (mbr must request a waiver to receive BAH at the rate of the old PDS).
