In Asymptomatic Patients of Primary Parathyroidism, Surgery is recommended in:
- Serum Ca> 1mg/dL above upper limits of Normal
- GFR <60 mL/min
- > 2.5 SD Below peak bone mass, T score < 2.5 on bone densinometry
- Age <50 Years
- Medical Surveillance no possible or desired by patient
What is the Chvostek Sign
Tapping on the Facial Nerve, anterior to the ear causes contraction of the ipsilateral facial muscles
- Sign of Hypocalcemia
What is Trosseau Sign?
Carpopedal spasm after occlusion of Blood flow to the forearm
- Sign of Hypocalcemia
Painful, violaceous lesion on the extremeties that may necrose and become gangrenous, leading to sepsis and death
Calciphylaxix
Increased PTH in response to Hypocalcemic states (Chronic renal failure, inadequate calcium intake, gut malabsorption)
Secondary Hyperparathyroidism
Increased PTH from abnormal parathyroid glands with Symptomatic patient management is Parathyroidectomy
