vasoconstrictors Flashcards
5 contents of an LA carpule
La agent
vasoconstrictor
antioxidant
preservative
ph adjusting agent
what is the antioxidant thats usually in a carpule
sodium metabisulfite (allergies)
what is the preservative that is usually in carpules (multidose only)
methylparaben (can cause allergies too)
plain local anesthetics are … by nature
vasodilators
how much max epi in CV patients
0.04 mg (2 cartridges of 1:100,000 epi)
what happens when you give a VC with alpha blockers (such as phenothiazines and butyrophenones)
hypotension and tachycardia
what happens when you give VC with non-selective beta blockers
hypertension and bradycardia
what happens when you give VC with a selective beta blocker (b1)
no effects since a1 will increase BP and b2 will decrease
what happens when yo u give VC with tricyclic antidepressants (amitriptyline/ elavil)
exogenous epinephrine may produce exaggerated effects since TCAs block reuptake of epi and norepi
are patients on TCAs contraindicated for VC
no but dont give more than 0.2
what happens when you give VC with patients on MAO inhibitors (phenylzine/Nardil)
little change since epi is also metabolized by COMT
what happens if you give VC with amphetamines/cocaine?
exaggerated response.
Should wait 24 hours!!!
5 contraindications to using vasoconstrictors
severe CV disease (ASA IV)
acute MI
anginal episodes at rest (unstable angine)
cardiac dysrhythmias that are refractory to drug treatent
patient with uncontrolled hyperthyroidism
