what are common UTI causing pathogens?
E.coli in 70-95% of uncomplicated
Klebsiella pneumoniae and streptococci
what are the S&S of LUTI (i.e cystitis)
Dysuria Frequency Urgency Suprapubic pain Haematuria
pyelonephritis has all the same LUTI symptoms but what other symptoms are present?
fever Flank pain Chills/rigors Costo-verterbral angle tenderness N&V
how is suspected UTI investigated and what would be seen?
Urine dip
- leucocytes, nitrites and blood
MSU culture
- organisms
urine microscopy
- bacteria, WBCs and RBCs
post void residual
- detects urinary retention
Imaging i.e renal US or CT
- if suspected renal tract abnormalites
- if unresponsive to Tx
- recurring UTIs
what is the treatment of lower UTI?
oral nitrofurantoin or oral trimethoprim
females 3 day course and male 7
what is the treatment of upper UTI without sepsis?
oral ciprofloxacin or oral trimethoprim
7 days for both sexes
what is the treatment of urosepsis?
IV Gentamicin
if eGFR<20 then IV temocillin
what is the treatment for catheter related UTI?
single dose IV gent or single dose oral cipro
what are the symptoms of prostatitis? How is treated?
UTI like symptoms + pain in genitals, pelvis, lower back or buttocks
4 week course of oral ciprofloxacin
what are the complications of UTI?
sepsis
AKI
renal and peri-renal abscess
pyelonephritis
