What is the most comm causative organism in uncomplicated cystitis?
E. Coli
What can a patient with cystitis present with?
Dysuria, cloudy urine, frequency/ nocturia, urgenc, haematuria, suprapubic tenderness, mild fever
What is the treatment of uncomplicated cystitis?
3 day course of trimethoprim or nitrofurantoin
What features could mean a lower urinary tract would be classed as complicated?
Male, children, pregnant women, immunocompromised
How would treatment differ for a complicated compared to uncomplicated cystitis?
Treatment is 5-7 days instead of 3 for uncomplicated
Also cefelexin can be used in pregnancy
What is pyelonephritis?
An infection of the upper respiratory tract by bacteria travelling up from the bladder
What investigation would you do for suspected pyelonephritis?
Mid stream urine sample and send off for susceptibility testing
What is the treatment for pyelonephritis?
7-10 day course of co-amoxiclav or ciproflaxin
Why would the drugs we use to treat cystitis not be appropriate for pyelonephritis?
Cystitis drugs work locally but pyelonephritis drugs have systemic activity
What is asymptomatic bacteraemia?
When there is significant. Levels of bacteria in urine without accompanying symptoms of UTI
Why is it that asymptomatic bacteraemia is only treated in pregnancy?
Left untreated there is higher risk of premature labour and pyelonephritis
