Is something that could potentially harm someone or could cause an adverse effect on health.

worn or damaged equipement, furniture and flooring can cause slip and trip hazard resulting in sprains, bruing or fractures, or in being knocked unconscious

medical orother waste products not disposed of following correct procedured and poor levels of hygiene can result in the spread of infection and disease.

medicines - incorrect dose administered, wrong medication, unauthorised access to medication can have serious health consequences.
cleaning materials - incorrect stores or used can cause serious physical harm

stress and fatigue - due to long working hours, copying with challenging behaviour, violence, abuse from service users; bullying in the workplace.

Working Conditions
temperature - neither too hot or cold because it can cause dehydration and exacerbate conditions such as asthma.
noise levels too high can have long term effects on hearing.
travel - long distnce, traffic being away from home often causes stress tiredness.
working practices
Excessive working hours result in tiredeness or lack of concentration, leading to mistakes and accidents.
lack of supervision for new staff or those taking on new tasks - training and supervision for new staff or those taking on new tasks - training and supervision are needed to avoid accidents and injuries e.g. hoist incorrectly could cause serious physical injury for a patient ad the staff.
Lack of Security Systems
Door looks, alarm systems and monitoring of visitors all prevent unauthorised access by strangers ho may pose a threat of harm for individuals in care environments; security may not be in place or may due to the poor maintenance.
