Action
A
- Blocks the chloride pump
- Keeps chloride and the sodium in the tubule to be excreted in the urine, thus preventing reabsorption

Indications
A
- Treatment of edema associated with CHF, liver, or renal disease
- Monotherapy or adjunct for the treatment of hypertension

Contraindications
A
- Allergy to thiazides or sulfonamides
- Fluid & electrolyte imbalances
- Renal disease
- Lactation

Cautions
A
- SLE (lupus)
- Liver disease
- Pregnancy/lactation
- Diabetes or glucose tolerance abnormalities

Adverse effects
A
- GI upset
- Fluid & electrolyte imbalances: hypokalemia, hypercalcemia
- Hypotension
- Hyperglycemia
- CNS effects: tingling, drowsiness, numbness, confusion
- Alkalinized urine

Drug-Drug interactions
A
- Digoxin: increased risk of digoxin toxicity r/t potassium changes
- Antidiabetic agents: may need adjustment
- Lithium: risk of lithium toxicity, monitor levels closely
