Sodium Fluid Balance Flashcards
Osmolality vs Osmolarity
Osmolality= mosm/kg
Osmolarity= mosm/L
What are the characteristics of Osmolality and Osmolarity?
Osmolality- lab calculated, accurate, unchanged by volume change
Osmolarity= more practical, easier to measure Ls of blood than Kgs
What is the Osmolarity gap?
The difference between lab calculated Osmolality and my Osmolality calculation
What causes an Osmolarity gap?
Additional solutes than Na, K, glucose and urea
Osmolality equation
2(Na + K) + glucose + urea
What should normal Osmolality be?
275-295 mosm/L
What value indicates an similar gap
> 10 mosm/L difference between lab value and my calculation
What regulates Na?
Blood volume and Osmolality
What happens when blood volume increases?
Atrial stretch, Atrial Natriuretic Peptide released, aldo renin adh decreased
What is the moa of aldosterone?
What is the Moa of renin?
What is the moa of adh?
What happens when blood Osmolality is high?
Thirst +Adh release
(Opposite for low Osmolality)
Step 1 of assessing hyponatraemia
Check plasma Osmolality
Need to know this is actually hyponatraemia!
What can cause euosmolar hyponatraemia
High lipids, proteins
Pseudohyponatraemia
