how do the sedative hypnotic anxiolytics Benzodiazepines work ?
enhance the inhibitory effects of GABA in the CNS Relief from anxiety occurs rapidly following administration
uses
generalized anxiety disorder and panic disorder, seizures ,insomnia,muscle spasm alcohol withdrawl, induction anesthesia , amnesic prior to surgery procedures
why is it given
decrease amount of anesthesia needed during surgery
its also used in moderate sedation meaning pt is
still arousable like in procedures like endoscopy, colonoscopy,joint dislocation
chemical restraint –
document restraint if used to calm the patient down
there is a difference in documenting
used as med for anxiety or to calm pt down and use as restraint
Complications , side effects related to med:
CNS depression : sedation ataxia , decreased cognitive function
what is ataxia
decrease in balance
anterograde amnesia
occurs after
Risk for Toxicity :
decrease Resp decrease Blood pressure ,
for toxicity with these medications give antidote :
Flumazenil
Paradoxial response
insominia , excitation,euphoria, anxiety rage
Withdrawl effects
opposite effects , anxiety insomnia, diaphoresis , tremors light headedness
what should you tell pt who may experience withdrawl effects ?
they are not common because these medications are used for short term so they shouldn’t experience these effects but should notify the provider if they are experienced .
to avoid withdrawl benzodiazepines
taper dose over several weeks
Benzo diazepines are generally used :
Short term due to risk for dependence
use cautiously in patients with
liver disease substance abuse disorder
grapefruit juice interaction :
can reduce metabolism and med stays in body longer
avoid fatty foods :
they decrease the absorption of the medication
This medicaton interacts with other
CNS depressants alcohol barbituates opiods
administration :
administer medication at bedtime because of cause of sedation with this medicaton, administer with meals or snacks if GI upset occurs
Benzodiazepines are contraindicated in clients who
have sleep apnea, rspiratory depression, or glaucoma
these meds are under what control
