What is it?

A
  • Chronic, systemic, granulomatous disease
  • Exaggerated T helper lymphocyte response –> granuloma formation and fibrosis

A
  • Abnormal XR (30%)
  • Fever, LOW, malaise, cough, dyspnoea (20%)
  • Arthralgia + EN (20%)
  • Eye - uveitis, sicca (10%)
  • Palpable LN (5%)
  • Hepatosplenomegaly
  • Skin - lupus pernio (5%)
  • Rare: cardiac arrhythmias, hypercalcaemia, CN palsies

Investigations (7)

A
  • Eosinophilia
  • ACE
  • ESR
  • CXR - cavitation, aspergillus
  • CT - GG, exclude TB
  • RFT - ILD, decreased VC, decreased DLCO, normal FEV1/FVC ratio - can get mixed obstruction
  • Biopsy via bronch
