Mark vehicles..

A

-location of tires (double wheeled–outside tire only) of ALL vehicles involved marked with “T”–outer edge of each tire and center line of axle
in addition location of four corners of vehicle, tractor trailer, 6 corners
-use chalk or lumber crayon, or anything available and RENEW as necessary until arrival of SAFETY BATTALION

–WITHIN 10 DAYS and a copy sent to Safety Command-Collision Recording unit within 96 hours of collision resulting in DEATH, PERSONAL INJURY, DAMAGE OVER 1,000$ to property of any one person, or dollar amount cannot be estimated

A
  • -Filled out by OFFICER in charge of apparatus at the time of collision/incident
  • if Officer or operator is injured , another member at scene shall gather info for CD-19, and the CD-19 should be prepared by an OFFICER ASSIGNED TO UNIT involved
  • if additional info is pertinent , such info should be entered on SF-1(supplemental form)
  • notation at top right ADDITIONAL INFORMATION, filled in only if necessary to attach PLAIN sheet OF PAPER CONTAINING ADDITIONAL INFO
