- significant damage to Dept. or civilian vehicle
- significant damage to private property
- life threatening injury or death to civilian or member

- apparatus fire
- damage to apparatus caused by unusual occurrence (tree limb falls on apparatus
- damaged discovered during routine inspection
- damage to civilian vehicle caused by properly parked apparatus operating at alarms, inspection , MUD (outrigger strikes parked car)
- properly parked apparatus STRUCK by another apparatus (INCIDENT FOR APPARATUS STRUCK/ COLLISION FOR APPARATUS STRIKING)

Who determines if the collision is MAJOR?
BC
FOC-1…
BC responsible for conducting investigation should email FOC-1, followed by phone call to FDOC
Cases where BC was not dispatched for an investigation(out of city/fire civilian incident(not requested)..
preparer of CD-19 (OFFICER) will contact FDOC with particulars
Fire/Civilian accident BC responds if REQUESTED
FIRE/FIRE–automatic BC response
MAJOR collisions, DO NOT move vehicles…except..
- to PREVENT additional collisions and or injuries at a collision scene
- fact that ONE lane of traffic is blocked does NOT warrant moving vehicles
Mark vehicles..
-location of tires (double wheeled–outside tire only) of ALL vehicles involved marked with “T”–outer edge of each tire and center line of axle
in addition location of four corners of vehicle, tractor trailer, 6 corners
-use chalk or lumber crayon, or anything available and RENEW as necessary until arrival of SAFETY BATTALION

96 HOURS of incident/accident

-prepare COLLISION PACKET send through CHAIN of command included: 2 copies of CD-19 2 COPIES OF MV-104 2 copies of police accident report
Original MV-104 COMPLETED AND SENT TO DMV..
–WITHIN 10 DAYS and a copy sent to Safety Command-Collision Recording unit within 96 hours of collision resulting in DEATH, PERSONAL INJURY, DAMAGE OVER 1,000$ to property of any one person, or dollar amount cannot be estimated

- -Filled out by OFFICER in charge of apparatus at the time of collision/incident
- if Officer or operator is injured , another member at scene shall gather info for CD-19, and the CD-19 should be prepared by an OFFICER ASSIGNED TO UNIT involved
- if additional info is pertinent , such info should be entered on SF-1(supplemental form)
- notation at top right ADDITIONAL INFORMATION, filled in only if necessary to attach PLAIN sheet OF PAPER CONTAINING ADDITIONAL INFO

- collision/INCIDENT involves MORE THAN ONE APPARATUS
- MORE THAN ONE CIVILIAN VEHICLE
- MORE THAN 2 PEDESTRIANS OR BICYCLISTS
* motorcycle is considered MOTOR VEHICLE)
