What do SABA’s do?
Cause bronchodilation and decrease mucous secretion
Act as agonist on B2 adrenoreceptors
What are common side effects of SABA’s
Tremors
Can cause abnormal heart rhythm
What is the first line treatment for asthma
Long term
SABA + ICS
What is the second line treatment for asthma
Long term
SABA + ICS + LABA / LAMA
What would you do if there is no response to second line treatment of asthma
Stop LABA, increase ICS
What would you do if there is some response to second line treatment of asthma?
Continue LABA, increase ICS
What is the third line treatment of asthma
Long term
SABA + ICS + LABA
Also + LTRA / Theophylline / Chromone
What is the fourth line treatment for asthma
Long term
Add oral steroid
and anti-IgE / anti-IL5 / anti IL4a
On top of other treatment lines
What is the treatment of acute asthma presentation
O SHITMAn
Oxygen (least 60%)
Salbutamol (neb)
Hydrocortisone (IV)
Ipratropium (neb)
Theophylline (oral)
Magnesium sulphate (IV)
An anesthetist (to intubate)
What do LABA’s do
Cause bronchodilation and decrease mucous secretion

Side effect of LABA
Can worsen asthma is given alone
Always given with ICS
What do ICS do
Reduce airway inflammation
Encodes anti-inflam proteins and inhibits inflam protein encoding
ICS side effects
Hoarse voice
Oropharnygeal candidiasis
Can cause pneumonia in patients with COPD
What do LTRA’s do
Prevent bronchoconstriction, mucus secretion and oedema
Stop CysLT’s from causing this
Side effect of xanthenes
E.g. Theophyllines
Nausea
Vomiting
Abdominal discomfort
Headache
If given too much can cause - dysrthymia, seizures and hypotension
