stroke, most patients die within a month, complications of immobility/brain lesion
Risk Factors for CVA
greater than 55 yrs
males

Stroke Rehabilitation Goal
to return maximal physical, psychology, cognitive function to improve quality of life/dignity of patient
Need for interdisciplinary Approach to Stroke Rehab
stroke is a multi dimension problem,
Hemiparesis
weakness on one side of the body
Speech Therapy
assesses speech, cognitive assessment, swallow evaluation
Physiatrist
coordinates the multidisciplinary team; provides primary medical care
Natural Recovery from Stroke Impairments
hemiparesis - common symptom in acute stroke patients
Progression of Sx after stroke
Twitchell
initial decrease MSR, then increased along with spasticity, then increased voluntary movement with time
Where does motor fxn return first…proximal or distally? LE or UE?
lower extremities, and proximal
Complete paralysis of hand prognosis?
poor
Some movement in hand at 4 wks prognosis
good prognosis
Prognosis of proximal spasticity
poor prognosis
CVA Complications (Shoulder Pain)
most developed during the spasticity phase, 75% of pts have hemiplegia
DDx for Shoulder Pain Complications
Complex Regional Pain Syndrome
Subluxation
Apraxia? Area lesioned?
difficulty planning and executing voluntary movement
left anterior corpus collosum
Aphasia
difficulty producing or comprehending speech
Orthosis
external device applied to body part that affects biomechanics
Walkers Indicated for? Adv? Dis?
used for bilateral weakness, poor coordination of LEs, relieve weight bearing on LE
Adv: greater support/stability
Disad: interferes with smooth gait pattern
