Pulmonary Hypertension
Define
Pulmonary blood pressure above 20mm Hg
Passive Pulm HPT
Cause: raised left atrial pressure
Due to: Mitral stenosis, LV failure, LA myxoma
Results: Pulm Oedema, reflex vasoconstriction, sclerotic changes in arteries & veins
Fix: mitral valve surgery
Hyperkinetic Pulm HPT
Cause: increase blood flow through lungs
Due to: ASD/ VSD
Obstructive PH
Cause: blockage of Pulm vessels
Due to: Emboli
Obliterative PH
Cause: Destruction of pulm vessels
Due to: Emphysema/arteritis/ pulm fibrosis
Vasoconstrictive PH
Cause: PO² < 60mm hg
Results: both lungs hypoxic due to redistribution
Idiopathic PH
Mostly woman 20-40years
Pathology: lesions of plexogenic pulm ateriopathy
Cor Pulmonale
Hypertrophy of RV
Cause: primary respiratory diseases
Excludes: hyperkinetic PH and passive PH
