Secondary- Chronic Lung disease or LHD
Caused by recurrent PTE
Congenital Heart Disease- shunt

A

Dyspnoea on exertion
Syncope/ Pre Syncope on exertion
Chest tightness

Raised JVP
Right ventricular heave
Loud second pulmonary heart sound
Hepatomegaly
Ankle oedema

Investigations

A

ECG
ECHO- detect fault in tricuspid valve
CXR- enlarged blood vessels
national pulmonary vascular unit- RH catheterisation, cardiac output.

Treatment

A

Treat underlying cause (HTN, lung disease)
Give anticoagulation and diuretics and O2
CCB
Prostacyclin
ERA (endothelium receptor antagonist)
CTEPH
lung/heart transplant.
