Hyperventilation syndrome - work up?
Common in the setting of panic attacks and anxiety disorders - get a CXR to R/out PE and pneumothorax
NMS vs Serotonin Syndrome
NMS has more rigidity while SS has hypereflexia and clonus
What is acute dystonia
Sudden muscle spasms - torticollis, retrocollis and trismus - caused by antipsychotics (anticholinergic effect)
Akathisia
Restlessness, agitation and inner tension
Tardative dyskinesia
Mouth smacking, tongue movements classically - but can be marked by any abnormal movements of the body
Side effects of Clozapine
Metabolic - weight gain, diabetes and dyslipidemia EPS effects and myocarditis
Medication with highest EPS?
First gens - haloperidol, Risperidone the most of the next gens
Non-typable HiB causes
recurrent otitis media
First generation antipsychotic action
D2 binding (high potency - haldol, low potency - Chlorpronazine)
Second generation (atypical) antipsychotic mechanism of action?
D2 binding - but less strong, also binds 5HT2A
Third generation antipsychotics mechanism of action
Partial D2 agonist
Biggest SE with atypical antipsychotics?
Weight gain / metabolic syndrome especially clozapine/ olanzapine
Dopamine and prolactin relationship
Dopamine inhibits prolactin, so antipsychotics can lead to prolactin hypersecretion, switch med/ reduce the dose
Tx for too much prolactin on antipsychotics
Bromocriptine, or amantadine
Complication of antipsychotics in young people?
Bone mineral density loss - osteomalacia, give Vit D and encourage good lifestyle
