A: kortdurend, beklemmende pijn retrosternaal. luxerende factor: inspanning/emoties
LO: niet specifiek
D: ECG, fietsergometrie, myocardscintigrafie coronair angiografie
bh: stop roken, afvallen, beweging, cholesterol medicatie, RR medicatie, bb
Instabiele ap
A: progressieve AP, frequenter, soms in rust
LO niet specifiek
D: ECG, coronairangiografie, hartenzymen
Bh: zelfde als ap, coronair angio: PTCA+stent, aspirine, bb, cabg
Myocardinfarct
A: langdurige pob, vegetatieve verschijnselen
LO: RR daling, crepitaties basale longvelden
AO: ECG, hartenzymen, coronair angio + PTCA
bh: thrombolyse, PTCA+-stent, antistolling, cholesterolverlaging, preventie met ACE remmer
aneurysma dissecans
A: acute heftige beklemming, pob + rug tussen schouderbladen, scheurend karakter, pijn meer bij ademhaling
LO: heftige pijnlijke pt, shock
AO: CT, slokdarm-echocardio-gram, mri
bh: chrirugie, plaatsen prothese
pericarditis
A: recente virale infectie, pob middenvoor, in epigastrio, schouder + nek, houdingsafhankelijk
LO: pericardwrijven, percussie: vergrote hartgrootte
AO: X: vergroot hart, echo, ecg: ST in alle afl. evt virusserologie
bh: pijnbestrijding, expectatief, corticosteroiden, pericardectomie
longembolie
A: recente vg: immobiliteit, familieanamnese, medicatie: OAC. Pijn vastzittend aan ademhaling, kortademig
LO: nie specifiek tot circulatoire instabiliteit tot plotse dood, pleurawrijven
AO: xthorax: wigvormig infiltarat, ventilatie perfucsiscintigram, spiraal ct thorax, ecg: tekenen rechtsbelasting, MRI: uitsluiten
Bh: heparine, oac, profylactisch antisolling bij risicopatient
pneumothorax
A; plotse pijn + kortademigheid, vast aan ademhaling
LO: trachea/hart ➛ hypersonore percussie en afw ademgeruis
AO: x
Bh: thoraxdrain, pleuodese: chemisch/chirurgisch
reflux oesofagitis
A: dyspepsieklachten, retrosternaal branden, nachtelijk ehoest
LO : niet specifiek
AO: oesofagogastroscopie, pH meting, manometrie
bh: voedingsadviesen, houdingsadviezen bij hd, zuurremming: H2 antag, ppi
thoraxwandpijn
A: trauma, hoestbui, huidafwijkingen
LO: palpatie ribben/wk, costochondrale gw: huidafwijkingen zoster
AO: symptom therapie: analgetica evt antivirale therapie = ook bh
pleurale pijn
A: pijn locaal vastzittend aan ademhaling
LO: pleurawrijven, tekenen van long-infiltraat
AO: x trhorax, evt CT
bh: analgetica, ab, antistolling, anti tumor bh
functionele klachten
A: pijn diffuus over de borst, kortademig, tintelingen, misselijk, hoofdpijn
LO: niet specifiek, hyperventilatie
AO: ecg/x ter uitstluiting, art bloedgassen, resp alkalose
bh: geruststelling, sedativa: kort, psychotherapie, gedragstherapie
