Partial Denture Problem Solving Flashcards
How can support be improved in the free end saddle?
Optimum extension
What are the problems with free end saddle?
Support
Stability
Retention
Abutment tooth damage
How can stability be increased in the free end saddle?
Extend into retro mylohoyoid space
How can rention be Increaed in the free end saddle?
Using direct and indirect retention
How can abutment damaged be reduced in free end saddle?
RPI
What is the RPI system?
Studies have shown that for free end saddles, if they have occlusal approaching clasp and distal rest seat then this causes more load onto the abutment tooth and little load to mucosa
Thus…
To get a more favourable loading style
- Mesial rest
- Distal guiding plate
- I shaped clasp
Hi does the RPI system work?
The minor connector carrying the mesial resin contact the mesio lingual surface of the abutment tooth and together with the distal plate reciprocates the tip of the clasp
Where is the clasp tip positioned on RPI?
Anterior or midpoint of the buccal surface if the premolar
What happens to the guide plate when the denture fully seated and during loading?
It moves the distal surface of the distal guide plane on the 4 and contacts the lower part of the guide plane
During loading the denture rotates about the a point close to the mesial rest and this means both the plate and I bar move away from the tooth this less stress put onto the tooth
What is the effectiveness of the RPI dependant upon?
Good technician
Make sure adjusted properly to allow disengagement
Where can problems arise during denture construction?1
At any stage Primary imp Sedondary imps Material Special tray Survey and design Storing problems Bite reg
What should you do for CoCr dentures before bite reg?
Assess the fit of the framework
What can cause looseness of the partial?
Usually Over extension But can be: Under extension Clasps Occlusion Poor fit
What can cause pain?
Over extension Under extension Clasps Movement Occlusion
How can you manage pain?
Adjust of consider remake
