  1. Intraductal
    - choledocholithiasis
    - FB
    - ascaris lumbricoides
  2. Intramural
    - malignant strictures (cholangiocarcinoma)
    - benign strictures (chronic pancreatitis, PSC, HIV-cholangiopathy, iatrogenic bile duct injury)
  3. Extraductal
    - periampullary malignancy
    - pancreatic pseudocysts
    - malignant portal lymphadenopathy
    - benign portal lymphadenopathy
