What is neurogenic bladder?
Condition that causes problems with bladder control
Most often happens in people who have a spinal cord injury, MS, or CVA
What are symptoms of neurogenic bladder?
Urinate small amounts more often than usual
Have problems starting to urinate or emptying their bladder
Lose bladder control
Have problems knowing if their bladder is full
Have repeated UTIs
For neurogenic bladder, attempt to work on
Bladder retraining - going to bathroom at scheduled times you make yourself go every hour even if you feel like you don’t need to and then you increase the amount of time you wait between bathroom visits
Pelvic floor muscle training
How is neurogenic bladder treated?
Oxybutynin to help relax the bladder
Catheter
What two functions of the bladder does spinal cord injury affect?
Storage and emptying of urine
May need CIC every
4 hours, bladder volume of less than 500 mL of urine is targeted in order to avoid bladder distention and infections
For neurogenic bladder, what study should you do?
Urodynamic study
What medications can be used for neurogenic bladder?
Oxybutynin and Tolterodine decrease bladder tone, suppress bladder contractions, and may reduce urinary frequency and incontinence
After spinal cord injury, sexual dysfunction can occur and which medications may need to be prescribed?
Sildenafil, Vardenafil, or Tadalafil
For a patient with a history of neurogenic bladder, what questions do you ask?
Recent infections?
Hematuria?
Flank pain?
What do you check with a patient with neurogenic bladder at each visit?
PVR
If neurogenic bladder patient is symptomatic can consider ordering
Urodynamic study
