What is bisphosphonate?
Drug that slows/ prevent loss of bone density
Most common scenario for pt to be on bisphosphonate?
Osteoporosis
Cancer
Paget’s
When are bisphosphonates given IV?
Metastatic disease

Alendronic acid
Zolendronic acid
Disdoium pamidronate
Risedronate sodium
How do bisphosphonates work?
Reduction bone turn over via effect osteoclasts
Chemical composition bisphosphonte?
Carbon core
2 x phosphonate groups
2 x side changes = R group
Purpose phosphonate group
Bind calcium - bind bone
Purpose of R group?
Toxic to osteoclasts

Pre-tx dental assess

Management of MRONJ
Specialist centre - difficult
Why MRONJ occur?
Anti-angigonesis = compromised blood supply
Direct toxicity to cells in bone = necrotic
