Depression first line
SSRI such as Sertraline, citalopram, fluoxetine, paroxetine
Citalopram interactions
Drugs that prolong QT interval (such as fluconazole)
Antidepressants and seizures
Antiepileptics lower seizure threshold
Antidepressant choice: warfarin
Avoid: SSRIs, SNRIs, TCAs. Try Mirtazepine, reboxetine, trazodone, mianserin.
Antidepressant choice: Triptans
Avoid SSRIs
Antidepressants that cause sexual dysfunction
SSRIs
Antidepressants that cause hyponatremia
All, highest risk with SSRIs
SSRI CIs
Manic phase bipolar, poor epilepsy control, severe hepatic impairment (sertraline), QT prolongation ( (es)citalopram)
SSRI warnings
Bleeding disorder (particularly GI), history of mania, cardiac disease, diabetes, epilepsy, hepatic impairment, renal impairment (sertraline), significant bradycardia ( (es)citalopram).
SSRI ADRs - Cardiac
Palpitations, tachycardia, QT Prolongation, Torsade de pointes
SSRI ADRs - GI
Diarrhoea, nausea, dr mouth, abdo pain, constipation, vomiting, dyspepsia, microscopic colitis, pancreatitis. Melania, stomatitis, tongue/mouth ulcers, glossitis.
SSRI ADRs - Nervous system
Headache, somnolescence, parasthesia, tremor, hypertonia, dysgeusia, convulsions, speach disorders. SEROTONIN SYNDROME
SSRI ADRs - Psychiatric
Insomnia, depression, nightmares, agitation, anxiety, suicidal thoughts and behaviour.
SSRI ADRs - Bone, soft tissue, skin
Bone fractures (unclear mechanism), rash, hyperhidrosis, oedema, alopecia, dermatitis
