21 yr old F, injured left knee, twisting injury, felt a pop, unable to play on, swollen the following day, clicking sensation in knee, locked twice
look
antalgic gait
swelling of joint
feel
patellar tap test for effusion
knee joint line tenderness
active and passive movement
decreased ROM due to swelling in early stages
resisted movement
nil of note
special tests
meniscus exam
cruciate ligament exam
medial and lateral collateral ligaments
meniscus exam
McMurrays test
cruciate ligament exam
post drawers test
ant drawer test
medial and lateral collateral ligaments
valgus and varus stress tests
DDx
cartilage/ meniscus injury
ACL rupture
PCL rupture
why is meniscal injury likely?
typical hx of locking and effusion a day after injury, pos McMurrays test
why is ACL rupture unlikely?
no hx of pop/ immediate SWELLING
NEG ANT DRAWERS TEST
why is PCL rupture unlikely?
much less frequent then ACL
usually fall or blow to flexed knee, post drawers test likely
investigations
no need but
xray/ MRI can show tear in meniscus
non-op tx
rest ice elevation
analgesia
physio
