What are the features of median nerve injury? ( supracondylar fracture of humerus and ai nerve and median nerve get injured)

A

All forearm flexor muscles of anterior compartment are paralysed.
flexor carpi ulnaris is normal so have ulnar deviation of wrist
FDS and lateral half of FDP (AIN) are paralysed.

Pronation lost ( p:teres. and p:quadratus(AIN)

Paralysis of loaf muscles: Thenar eminence atrophy

Loss of opposition of thumb: lead to Ape thumb deformity

Sensory loss over
Thenar eminence ( palmar branch affected)
Palmar surface of lateral 3.5 finger loss of sensation

A

During median and AI nerve injury ,
FDS and lateral half FDP ( ain) is paralysed

So
Loss of flexion of interphalangeal joints of midde and index finger , it is called Benediction hand deformity

What is overlapping and pointing index deformity?

A

Overlapping is as a result of paralysis of lateral half of FDP ( by AIN ) rarely the ulnar nerve supplies the tendon to middle finger which result in loss of flexion of IP joint of ONLY THE INDEX FINGER which is called POINTING INDEX DEFORMITY

It is the median nerve injury at the wrist ( lower part of forearm)

Features

Paralysis of LOAF muscle
1.thenar eminence atrophy
2.ape thumb deformity
Loss of sensation over thenar eminence ( palmar branch affected)
Loss of sensation over lateral 3.5 digit on palmar side.

A

Compression of median nerve while passing BELOW FLEXOR RETINACULUM BAND.

Features are SAME as that of wrist slash injury EXCEPT

Sensory supply of thenar eminence is normal b/c palmar branch of median nerve pass above the fr band
