Difference between typical carcinoid and atypical carcinoid

According to WHO criteria, how do you differentiate typical carcinoid from atypical carcinoid?

A

Note: only applicable in resected surgical specimens
Note: Ki-67 is not part of the WHO criteria
Answer: mitotic rate and presence of necrosis. If > 2 per 2 mm2 then atypical

however, typical carcinoids generally 1-5% Ki-67 and aytpical 5-20 to 30
