Requirements for filing out IOR
Irregular Operations Report
list of events that require IOR is in FOM 12-30
IOR’s should provide sufficient information so that a third party, such as a regulatory agency, can clearly identify the reason the report is being filed.
Types of alternates
Dual alternates 3585
Takeoff alternate (must be within 400)
1-2-3 rule
Supplement Operations

1-1-3 rule
Flag operations greater than 6 hours
1-1-2 rule
For at least 1 hour before and 1 hour after ceiling will be less than 1000 and visibility will be less than 2 miles.
(Cannot be used for thunderstorms)
Destination airport must have a suitable CAT 2 or 3 approach available.
May not be used with 3585.
Must be labeled on flight plan.
1-1-3 rule
1 hour before and 1 hour after ceiling will be less than 1000 feet and visibility will be less than 3 miles.
Destination must have a CAT 1 ILS available for use.
May not be used with 3585.
Minimum fuel
Advise ATC and dispatch of a minimum fuel state by declaring minimum fuel when having committed to land at specific airport. Any change to the existing clearance may result in landing with less than final reserve fuel.
Emergency Fuel
When the calculated fuel on landing at the nearest airport where a safe landing can be made, will be less than the planned final reserve fuel (30 minutes)
Forecasted/impending turbulence
Forecasted: make PA flight attendants take your jumpseats.
Turbulence improves/subsides: make pa flight attendants check in.
Unexpected turbulence of moderate or greater
Moderate or greater turbulence: make pa flight attendants be seated immediately, be seated immediately, be seated immediately.
Turbulence improves: make pa flight attendants take your jumpseats
Turbulence ceases: make pa flight attendants check in.
First officer limitations less than 100 hours as a United FO
Special qualification airport
RVR more than 4000 feet or visibility more than 3/4 mile
Maximum crosswind component 15 knots
Braking action good or better
No runway clutter
No reported wind shear
First officer takeoff and landing minimums with 100 hours or more in type
Takeoff: touchdown RVR and Rollout RVR 1000 feet (mid can be substitute for either)
Landing: RVR 1800 feet or visibility 1/2 mile
When do you need a takeoff alternate?
When weather conditions at departure airport are below landing minimums. Takeoff alternate must be located within 400NM from departure airport.
What weather is required at alternate to make it legal?
Airports with one operational approach facility add 400/1
Airports with two operational approach facilities add 200/1/2
What speed is bugged for takeoff?
V2
What speed does SRS command (2 engines)
V2 + 10
What speed does SRS command in single engine?
Speed existing of the time of engine failure no lower than V2
