Lecture 14 4/1/25 Flashcards
What are the characteristics of lagophthalmia?
-failure to blink completely
-can occur in patients with buphthalmia
-schirmer’s measurements may still be normal
-can lead to development of keratitis
What is flare pathognomonic for?
uveitis
What are the characteristics of synechiae?
-pathognomonic for uveitis
-indicate some degree of chronicity
What are the characteristics of iris bombe?
-sequence of events that starts with the pupil edge forming synechiae to the anterior lens capsule for 360 degrees
-aqueous humor becomes trapped in posterior chamber because it cannot get through occluded pupil
-iris billows anteriorly, causing iridocorneal angle to close
-impaired aqueous outflow causes increase in IOP
What are the characteristics of secondary glaucoma?
-glaucoma that occurs secondary to another problem
-“problem” must be something that impedes outflow of aqueous humor, such as uveitis
How does uveitis cause secondary glaucoma?
-uveitis can cause iris bombe
-uveitis can cause pre-iridal fibrovascular membranes (PIFMs), which grow across the iridocorneal angle and slow fluid outflow
What are the characteristics of rubeosis iridis?
-reddish appearance to the iris
-can occur when blood vessels within the PIFM are large enough to be observed on exam
What can lead to secondary glaucoma besides uveitis?
-hyphema
-tumors that grow into the angle
-primary anterior lens luxations
-complication of cataract surgery
Why can keratitis be seen in glaucoma patients?
due to lagophthalmia caused by buphthalmia
Why is surgery the better option to providing pain relief for buphthalmic dogs with secondary glaucoma?
secondary glaucomas do not tend to respond well to medical therapies
Why is timolol not used in secondary glaucoma cases?
-there is no evidence that preemptive glaucoma therapy delays onset of secondary glaucoma
-not every uveitis eye develops glaucoma
What is the treatment for anterior uveitis?
prednisone acetate and atropine therapy
