what are possible systemic complications of LA

A

psychogenic/ stress - common in anxious ptx , palpitations cold sweats etc , manage by lying flat with legs raised perhaps a sugary drink
in pregnancy avoid felypressin
drug interactions - consult BNF
toxicity - overdose

why might prolonged anaesthesia occur

A

direct trauma of nerve from needle

chemical trauma - injection given directly into nerve

why might trismus occur when giving an LA injection

A

damage to medial pterygoid muscle
may need muscle relaxant
will present within a few hours but may last weeks/months

why might facial palsy occur when giving LA

A

IDB injection enters parotid gland and anaesthitises facial nerve
all facial muscles on that side will be affected
will present within minutes and will get better in around 2 hours once LA wears off
