Before giving someone fluids outline in your head the following:
- What is wrong that causes the patient to need fluids?
- What are your goals in giving this patient fluids?
- What will you do net if fluids don’t help?
- What will happen if you don’t give the patient fluids?
What does NS contain?
154 mEq Na and 154 mEg Cl
What does plasmalyte contain?
140 mEg Na, 5 mEq K, 3 mEq Mg, 98 mEq Cl, and 27 mEq Acetate
What are types of colloids?
blood products, albumin, hextend
What are examples of blood products?
PRBC, FFP, platelets, cryoprecipitate
When do we use blood products?
acute bleeding, symptomatic anemia, coagulopathy, DIC
Why is albumin used?
Its a volume expander that increases intravascular oncotic pressure in order to help keep fluid intravascular.
When do we use albumin?
hypovolemia w/suspected or proven hypoalbuminemia (causes: burns, malnutrition, ARDS, drainage of protein laden pleural effusion or ascites, liver dysfunction)
What are cautions to using albumin?
oliguria or renal failure not caused by hypovolemia
What is hextend?
completely synthetic, hetastarch containing volume expander in a lactated electrolyte solution that resembles normal plasma
When do we use hextend?
hypovolemia as a synthetic alternative to albumin
What are cautions with hextend?
oliguria or renal failure not caused by hypovolemia, may exacerbate bleeding disorders
