ID AntiFungals Flashcards
Azoles Prototype Drug
Fluconazole
Azoles MOA
Inhibits fungal wall synth via inhibition of ergosterol synth. CP450 enzyme inhibitor
Azoles AE
Hepatotoxicity(rare)
Echinocandins Prototype Drug
Caspofungin
Echinocandins MOA
Weakens cell wall by inhibiting beta-(1,3)-glucan IV Exposes beta-glucan—-> exposes antigens for immune recognition
Echinocandins AE
Elevated liver enzymes. Vomiting, diarrhea
Griseofulvin Prototype Drug
Griseofulvin
Griseofulvin MOA
Inhibits mitosis by binding tubulin and tubular proteins required for cell division
Griseofulvin AE
CNS: Headache, peripheral neuropathy, sleep disturbances.
Dermatological: skin rashes (urticaria, lichen plants, erythema, photosensitivity, edema). Neutropenia (rare)
Polyenes Prototype Drug
Amphotericin B
Polyenes MOA
Nystatin: topical, Creates porous channels in fungal cell wall for hydrogen and potassium leakage by binding to sterols (Ergosterol, cholesterol)
Polyenes AE
Amphotericin B: nephrotoxicity, infusion reactions(fever, chills, hypotn, vomiting, dyspnea, headache), electrolyte abnormalities(hypokalemia, hypomagnesemia)
Pyramidine Antifungals Prototype Drug
Flucytocine
Pyrimidine Antifungals MOA
na synth by inhibiting thymidylate synthase.
2. Prevents protein synth by incorporating into fungal RNA as 5-FU
Pyrimidine Antifungals AE
GI, infection, Liver damage, toxic epidermal necrolysis
Bone marrow suppression (rare)
