how can mood be monitored?
by asking the patient how they feel on a scale of 1 to 10
how can cognitive function be monitored?
ask to remember 3 objects then have a conversation and ask them to say the 3 objects aloud
what three questions should be asked for insight?
are symptoms due to illness?
is this a mental illness?
do they agree with the treatment plan?
how do you ask if someone hears voices?
“I would now like to ask you a question which we ask to everybody. Do you ever seem to hear noises or voices when there is no one about and nothing else to explain it?”
what is a delusion ?
an unshakeable idea or belief which is out of keeping with the person’s social and cultural background; it is held with extraordinary conviction
- fixed idea out with a persons cultural background
what abnormal possession of thoughts are commonly reported in schizophrenia?

what are hallucinations?
Have the full force and clarity of true perception
located in external space
no external stimulus
not willed or controlled
what is hypnopompic phenomena ?
having hallucinations in thestate of consciousnessleading out ofsleep, (seen as normal)

what are the key symptoms of depression?
persistent sadness
loss of interests
fatigue
if any of the key symptoms of depression are present what other associated symptoms should be asked about?
disturbed sleep poor concentration low self confidence suicidal thoughts guilt
how many symptoms have to be present for mild, moderate and severe depression?
mild - 4
moderate - 5
severe - 7
