What is the triad assoc with Haemolytic Uremic Syndrome?
A
- acute renal failure
- microangiopathic hemolytic anaemia
- thrombocytopaenia

What triggers HUS?
A
a GIT infection with verocytotoxin producing E coli

What to look for in the hx?
A
- contact with farm animals eating uncooked beef
2. bloody diarrhoea- shigella

what is the pathophysiology
A
- toxin enters gut and localises to the kidney causing intravascular thrombogenesis
- coag cascade activated
- Plt consumed
- damaged RBC in circulation through occluded vessels

Management of HUS
A
Supportive therapy including dialysis

Is follow up required?
A
yes, look out for proteinuria and htn and ckd

Atypical HUS characteristics
A
no diarrhoeal prodrome
high risk of htn

Treatment of atypical HUS
A
eculizumab, plasma exchange
